In the future, Bob Arctor (Keanu Reeves) plays both a menacing drug lord and a cop who is after the drug lord, though he does not realize that he is both. Bob abuses a drug called Substance D which, after a long period of time, can split the personalities of the brain, causing the two to fight each other for control. Later, Bob learns of his new "disability" and is put to work in other fields.
